ABSTRACT

To analyze types of urinary calculi and patients' clinical characteristics, and to explore the strategies for prevention and treatment of urinary calculi.
Methods: A total of 1 849 patients with urinary calculi were treated in the Department of Urology, the Third Xiangya Hospital of Central South University. The components were analyzed by infrared spectroscopy. The relationship between stone composition and clinical parameters was analyzed according to the clinical characteristics of the patients.
Results: The proportion of calcium oxalate stone or uric acid stone in male (84.1% or 7.7%) was higher than that in female (78.4% or 4.2%). The older patients were more likely to be diagnosed as uric acid stone. The proportions of uric acid stone in patients <18 years old, 18-<41 years old, 41-<66 years old, and ≥66 years old were 0.0%, 1.6%, 6.6%, and 12.4%, respectively. There was no significant difference in the proportion of stones in patients with different BMI. There were no significant difference in the stone composition between the patients with or without urinary tract infection, hypertension or diabetes. The proportion of uric acid stones in patients with acidic urine was higher than the other types. The proportion of uric acid stones in patients with elevated creatinine (12.1%) was higher than that in the patients with normal creatinine (4.5%).
Conclusion: Elderly patients, or patients with high uric acid and renal insufficiency are more prone to uric acid stones. Regulation of urinary pH may be an important strategy for preventing and treating urinary calculi in Hunan Province.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the effect of bone regeneration with two different ratios of nano-hydroxyapatite(nHA)/collagen(Col) (nHAC) after tooth extraction in canine.</p><p><b>METHODS</b>Two kinds of nHAC grafts were prepared with different nHA/collagen ratio of 3∶7 and 5∶5. Eighteen male healthy adult dogs had been randomly divided into three groups. Immediately after extraction of the mandibular second premolars, each kind of nHAC was implanted into extraction sockets as follow: Group A, nHA/Col=3∶7(12 sites); Group B, nHA/Col=5∶5(12 sites); Group C, blank control group(12 sites). The bone repairing abilities of the two grafts such as vertical distance of alveolar, CT values, general observation, histological observation, trabecular volume fraction and porosity were separately analyzed at 1st, 3rd and 6th month, respectively (each group had 4 sites in different time periods).</p><p><b>RESULTS</b>nHAC were absorbed gradually after they were implanted into alveolar bone defect and were replaced by new bone. The vertical distance of alveolar bone in Group A([15.76±0.28] mm) was significant higher than that in Group B([14.88±0.36] mm), and CT values of Group A([879±31] HU) were higher than those in Group B([718±29] HU) (P<0.05). The trabecular bone volume percentage of Group A([22.2±0.4]%) was higher than that in Group B([20.3±0.4]%), and the bone porosity of Group A([23.6±0.9]%) was lower than that in Group B([27.6±0.6]%) (P<0.05). In addition, the vertical distance, CT values and the trabecular bone volume percentage of Group C was lower than those in Group A and B, but the bone porosity of Group C was higher.</p><p><b>CONCLUSIONS</b>The nHAC with nHA/collagen ratio of 3∶7 could better promote bone regeneration than nHAC with the nHA/collagen ratio of 5∶5 did.</p>

ABSTRACT

BACKGROUND:It is an important issue of alveolar ridge preservation after tooth extraction. Because of the limited treatment and geographical conditions, lack of awareness of oral health, many local people in frontier areas in Xinjiang have poor alveolar ridge that is not of timely repair after tooth extraction. Thus, it is important to find a suitable local and efficient way to save the residual ridge, which has a more practical clinical value. OBJECTIVE:To investigate the feasibility of nano-colagen artificial bone used for alveolar ridge preservation in the Kazakh in Xinjiang Tacheng Region, China. METHODS: Sixty-eight Kazakh patients with bilateral extraction from Tacheng region, Xingjiang Uygur Autonomous Region, China were selected in a self-controled trial. According to the principle of a minimum alocation imbalance index, the experimental side and control side of extraction patients were confirmed. In the experimental side, nano-colagen artificial bone was implanted; while, conventional treatment was done in the control side. Multi-slice spiral CT was used to scan the regions of extraction interest to measure the relative gray value of alveolar bone mineral density immediately and 3 months after implantation. RESULTS AND CONCLUSION:Immediately after implantation, tooth extraction sockets were visible on CT images both in the experimental and control sides; but after 3 months, the extraction sockets became unclear on the CT images, and CT values were close to those of the surrounding alveolar process, but bone tissues were ful of the bone graft area in the experimental side. The alveolar bone mineral density was higher in the experimental side than the control side at 3 months after implantation. These findings indicate that the nano-colagen artificial bone has good clinical achievement in alveolar ridge preservation in the Kazakh in Xinjiang Tacheng Region.

ABSTRACT

<p><b>OBJECTIVE</b>To determine if locally administered bone morphogenetic protein-2 (BMP-2) and osteoprotegerin (OPG) improved osteogenesis and new bone formation by trans-sutural distraction osteogenesis.</p><p><b>METHODS</b>Twenty four dogs were divided into three groups randomly and received new internal trans-sutural distraction osteogenesis treatment. Five days after operation, infusion apparatus with double-tube was inserted to submucosa near the distracted zone to deliver controlled release agent of recombinant human bone morphogenetic protein-2/poly (lactic-co-glycolic acid)/fibrin sealant (rhBMP-2/PLGA/FS) in group A and group C. Recombinant human osteoprotegerin/fibrin sealant (rhOPG/ FS) was injected three weeks later in group B and group C. Histology staining and bone histomorphometry were used to measure the changes of maxillary bone sutura after distraction for 1, 2, 4 and 6 weeks.</p><p><b>RESULTS</b>New bone formation observed in distracted zone showed a significant increase in group A and C. Transmission electron microscope showed the osteoblast and osteocyte were active with dilated rough endoplasmic reticulum and a large number of chondriosomes and Golgi complex. After distraction for 6 weeks, indexes of osteoblast of group A, B, and C were 38.5 +/- 7.7, 35.7 +/- 6.5, and 41.7 +/- 11.0, indexes of osteoclast (Ioc) were 5.9 +/- 1.0, 1.2 +/- 0.3, and 2.8 +/- 0.4, bone trabecula thicknesses were (38.36 +/- 13.28), (66.20 +/- 9.16), and (51.85 +/- 9.92) microm respectively. Increased bone density and decreased Ioc were found in group B and C.</p><p><b>CONCLUSION</b>The new elastic distractor is effective in inducing new bone formation. BMP-2 and OPG combination acts synergistically, and leads to significant enhancement of bone formation and remodeling.</p>

ABSTRACT

BACKGROUND: Muscle-derived stem cells (MDSCs) have been accepted as seeding cells in tissue engineered artificial nerves. Tacrolimus exhibits anti-immunologic rejection and promotes nerve regeneration and recovery. Whether can combine these factors with acellular nerve to form a new bridge that can inhibit immunologic rejection and promote nerve regeneration is uncertain. OBJECTIVE: Using the freeze-thawing combined optimized acellular nerve hypotonic-chemical detergent to prepare acellular nerveallograft scaffold. To explore the effect of co-application of MDSCs and FK-506 on nerve regeneration and function recovery following acellular nerve ailograft implantation.METHODS: The sciatic nerve derived from SD rats was prepared nerve bridge after acallular disposal. Gel containing FK-506 and MDSCs was injected into acellular nerveallograft scaffold with 100 μL microsyringe to repair defects. A total of 32 SD rats were randomly divided into 4 groups, with 8 animals in each group. Agap of 10 mm of left sciatic nerve was removed. And then the defects were repaired by extracted nerve graft containing FK-506+MDSCs, MDSCs and FK-506, respectively. In the control group, only hyaluronic acid gel was injected. Sciatic nerve function index (SFI) and electrophysiological exam were performed at weeks 8 and 12 after operation. Gross observation, neurohistological and ultrastructure were observed at week 12 after operation.RESULTS AND CONCLUSION: Compared with the same time point, the SFI, recovery rate of the motor nerve conduction velocities (MNCV), and myelinated nerve fibers in grafting part and in its distal part in the FK-506+MDSCs group were superior to other groups (P < 0.05). The nerve grafts were in normal size with considerabie blood vessels and slightly connected to peripheral Ussues. Compared to other groups, the regenerated nerve fiber in the FK-506+MDSCs group was more density with well-arranged order. A great quantity of Schwann cells proliferated in grafting. The density end diameter of myelinated fiber in the middle and distal part of the grafting were all greater than that of MDSCs and FK-506 groups, and there were few connective tissues between microfascicles, which was close to normal level. The co-application of MDSCs and FK-506 enhances peripheral nerve regeneration and functional recovery in acelluler nerve allograft graft. Therefore, MDSCs and FK-506 are synergistic factors in peripheral nerve injury repair.
